Our Strategic Plan
Integration & Partnerships
By continuing to foster relationships and working collaboratively with others, we can make the most of scarce human and financial resources to deliver the services our community needs and we can contribute to a better patient and family experience within the health system. The foundation is there. Trillium Health Centre already enjoys relationships with more than 400 health care and community-based organizations.
Integrated care is all about making sure that patients receive the right care at the right time in the right place by the right people. It involves learning to work and deliver care in a different way. It’s about health care professionals coming together to work in collaborative teams to generate improved patient outcomes. And it’s about seeking opportunities to work together to make more efficient use of available resources and to better meet patient needs.
Integrated Care means that:
- We purposely partner with others to help create an integrated and seamless health care system
- We foster inter-professional and inter-program collaboration
- We advance innovation in health care by partnering with government and leading public, private and academic organizations
