Attend our conference, Transforming Seniors’ Care: Within, Between and Beyond

Invitation to Attend

All individuals and organizations with a shared desire to improve seniors’ care, either within their organizations, between organizations or beyond, are invited to our two-day conference/think tank to improve seniors’ health today for tomorrow. The is conference, scheduled during Seniors’ Month, on June 23, 24, 2010, will bring together international, national and local experts and thought leaders to develop a strategy and action plan for integrated care systems for older adults within and between the various sectors of the health care system. Join confirmed speakers Dr. Jan McElhaney (Vancouver), Dr. Howard Bergman (Montreal) and Dr. Dennis Kodner (New York), as well as local experts, including Dr. Jane Barratt and Dr. Barbara Clive.

With Canada’s aging population, we recognize the need to focus on how health care providers serve the nation’s older adults. At Trillium Health Centre, this is a key opportunity in our three-year Strategic Plan. Canadians are demanding we improve access and service delivery through enhanced integration, coordination, communication and client-focused care. We are moving forward on our commitment to transforming how care is provided to seniors within our health centre, as well as between providers and beyond.

Located at 100 Queensway West in Mississauga, at the corner of Queensway West and Hurontario Street (Highway 10), just two minutes north of the Queen Elizabeth Way (QEW).

Directions by car:
From the QEW, east or west, take the Hurontario St. North exit. At the third set of lights, turn left on Queensway West. The Main Entrance and Parking, are all at the first set
of lights.

Visitor parking is available in a five-story parking garage located east of the main driveway.

Parking Fee: $2.50 per half hour to a maximum $14.00/day

One-Day (conference presentations, continental breakfast, lunch and refreshment breaks on day attending)
