Transforming the
Health Care Experience
Trillium Health
Centre is committed to working with our community partners
to create a more patient-centred experience for the people
we serve. A key part of achieving our vision – Together…Leaders
in Health Innovation – is the realization of a Community
Care Services Model (CCSM) that promotes a coordinated process
for our patients and their families, regardless of where they
receive care.
We believe everyone, from hospital and community-based health
care providers, to patients and families in their homes should
have the information they require to make timely and informed
decisions about their health. To make this possible, we must
collect, analyze and share information in new ways –
both within the walls of Trillium Health Centre and beyond.
That’s where Trillium’s THINK Initiative (Transforming
Healthcare into Integrated Networks of Knowledge) comes in.
The THINK Initiative represents a bold 7-year commitment that
Trillium is making in its people, its patients and its community.
We believe that, together, we can positively impact the health
of the individuals and families we serve by identifying new
and innovative ways to use technology to deliver information
and support safe and effective care. In collaboration with
our community, the THINK Initiative will:
• Provide patients and families with access to their
electronic health information to enable them to take a more
active role in managing their health and wellness.
• Provide integrated information across the continuum
of care to ensure our health care teams have appropriate,
accurate, and timely information for making decisions and
providing excellent patient care.
• Establish unique new relationships to deliver innovative
solutions and improve the health of the people we serve.
• Preserve our community’s current investment
in electronic-based systems while taking advantage of new
technology to enhance patient care.
THINK’s Four
Program Areas
The THINK Initiative is organized into the following four
Program areas.
• The Clinical Program will ensure that our providers
can deliver integrated patient-centred care to the patients
and families we serve by providing them with decision-making
tools and necessary patient information.
• The Business Applications Program will ensure that
we provide the right information, at the right time to all
staff to support decision making and strong analytical processes.
• The Learning and Knowledge Program will ensure that
we create and share knowledge and support learning in ways
that best meet the needs of our staff, physicians and the
communities we serve. It will build capacity that allows our
people and patients to learn in their preferred way, when
it’s convenient to them.
• The Infrastructure Program will build the technology
backbone that is required to support the Clinical, Business,
and Learning and Knowledge Programs and ensure the adoption
of new technology.
Trillium Alliance
for Care & Technology (TACT)
Supporting Trillium’s innovative approach to transforming
the patient experience are our eight partners, referred to
as the Trillium Alliance for Care & Technology (TACT).
Each of our partners is a global and recognized leader in
their respective domain. Our TACT partners include: IBM, Eclipsys,
Cognos, Courtyard, Sybase, Agfa, EMC, and IMS Maxims.
