Trillium Health Centre reports improved results in national patient safety indicator

Canadian Institute for Health Information releases HSMR data

Trillium Health Centre’s results  improved in relation to the national average in Hospital Standardized Mortality Ratio (HSMR) reports for 2009-10 issued by the Canadian Institute for Health Information (CIHI) today.

HSMR measures the ratio of actual to expected deaths in acute care hospitals by comparing the actual number of deaths in a hospital or health region with the overall Canadian experience - after adjusting for various factors that affect in-hospital mortality, such as the age, gender, transfers, diagnoses and admission status of patients.

Trillium’s HSMR results decreased significantly in fiscal 2009-10 to 85 compared to 97 the previous year, and well below the national average for expected mortality rate, which is calculated to establish the national average baseline ratio of 100.   

Trillium Health Centre is a strong supporter of the CIHI HSMR study because it is an important high-level statistical measure of patient outcomes over time.  HSMR is used by hospitals in a number of countries around the world to assess and analyze mortality rates and to identify areas for improvement in quality and safety of patient care.

"What we've seen across Canada is that the HSMR has been a great motivator for change," said Patti Cochrane, Trillium's vice president, patient services and quality and chief nursing officer. "Trillium has been able to use HSMR results to understand what is driving mortality rates and where improvements can be made. As a result, we've made tangible changes to the care we provide, making a real difference to patient outcomes and potentially saving lives."

Trillium uses many indicators to evaluate how our health centre is performing, both to identify areas where we are doing well and those where improvements could be made. This allows us to revisit or reinforce our approaches and processes to improve safety on an ongoing basis.

The introduction of Trillium’s Quality and Patient Safety Plan has reinforced our culture of quality and patient-centred care. The plan outlines Trillium’s framework for continuous improvement as well as a number of quality initiatives which have collectively contributed to a reduction in Trillium’s HSMR numbers. The effectiveness of such programs is tracked in relation to four main quality indicators, of which is HSMR is one.

A few examples of Trillium’s quality initiatives are listed below:

  • Medical Emergency Teams (MET)
  • Verbal order ‘read back’ process 
  • Physician order sets
  • Surgical safety checklists
  • Safer Healthcare Now! initiatives
