Current Quality Improvement Initiatives at Trillium
Tracking Quality Improvement
- Trillium’s award-winning ‘quality dashboard’
– an electronic display of key indicators that Trillium
uses to regularly monitor quality indicators for clinical/
service and process/operational outcomes – continues
to be enhanced. The dashboard places meaningful information
on quality indicators at the fingertips of all decision-makers
allowing them to interpret and react appropriately to variations
and to know when change is making a difference.
Patient Satisfaction
- The hospital has implemented a new patient satisfaction
survey tool in the past year that will deliver more timely
feedback on patient satisfaction relating to the Emergency,
Inpatient Units and Complex Continuing Care, allowing the
hospital to more accurately trend progress related to the
implementation of patient satisfaction initiatives
.
- The medical and surgical patient units have implemented
an initiative over the past year that focuses on addressing
patients on a more personal level by their name (i.e. Jane
or Mrs. Smith, etc.) Patients have indicated through past
surveys that this is important to them and the hospital is
tracking the impact this is having on the patient experience.
- Health care teams are meeting regularly to discuss what
they can do differently for their patients, and are asking
patients what has the most meaning for them in terms of their
overall hospital experience.
- The hospital has implemented ‘Staff Councils’
on patient units over the past year, which are lead by front-line
staff. The Councils are designed to allow staff the opportunity
to talk about issues important to both them and their patients
and to seek solutions that will bring about positive change.
Best Practice Projects
- The hospital has implemented a number of ‘best practice’
projects designed to support staff in finding ways to improve
care and services for patients. Among the projects that have
been completed or are currently in progress are: fractured
hips, stroke, falls prevention, cardiac surgery and cardiology
patient flow, paediatric day surgery, suicide assessment,
diabetes, pneumonia, total knee replacement.
- A pain management initiative under the total knee replacement
best practice project is a fine example of a project that
has successfully lead to improved out-comes for patients who
were previously experiencing severe pain with this pro-cedure.
The learnings from this pilot pain management initiative can
now be reviewed for application in other areas of the hospital
where pain management is an issue for patients.
- A project designed to achieve best practice in patient and
family stroke education lead to piloting an evening stroke
education program. A survey of stroke patients and family
members following the program pilot indicated an improved
family understanding of stroke as well as improved timeliness
of information for patients.
Community Consultation
- Trillium’s Community Advisory Council is a sub-committee
of the Board of Directors, where eight community representatives
from Mississauga and Etobicoke provide feedback on hospital
initiatives, programs and services, identify community issues,
and recommend solutions. Task forces and focus groups falling
under the umbrella of the Council allow for expanded community
input on specific topics.
Staff Support
- Trillium sent 17 staff and physicians to the IHI National
Forum on Quality Improvement in Healthcare in the Fall of
2003 to bring back learnings from their health care peers
around the world that could be applied to improve quality.
- 210 bursaries were provided to full and part time Trillium
staff for professional development.
- 1,000 staff attend the hospital’s annual ‘Back
To School’ conference in the Fall, which features a
line-up of exceptional speakers sharing their expertise on
enhancing both the staff and patient experience in hospitals.
- Handed out six Performance Excellence Awards (four team
and two individual), which recognize individuals and teams
in the hospital who have instigated a quality initiative designed
to enhance patient care.
