Ontario Health Quality Council highlights Trillium Health Centre as a Provincial Success Story in Managing Patient Flow

Partnership with Mississauga Halton CCAC and support from LHIN lowers number of alternate level of care (ALC) patients

Mississauga-West Toronto – Management of alternative level of care (ALC) patients at Trillium Health Centre (THC) is a provincial success story according to the Ontario Health Quality Council (OHQC) 2010 annual report. Released on June 3, the independent agency’s report acknowledges Trillium’s achievement in reducing the number of ALC beds hospital resulting in decreased wait times and improved patient flow.
 
“Our report shows that sixteen per cent of all hospital beds in Ontario are occupied by patients waiting for placement who do not need to be in hospital. As a result, the healthcare system is wasting resources,” said Dr. Ben Chan, CEO of the Ontario Health Quality Council.  “Trillium has made huge progress in managing this issue, and has many great lessons to share with other hospitals on how to improve.” 

The OHQC’s fifth annual report assessing the quality of the health system in Ontario found that roadblocks in patient flow to long-term care are causing serious backlogs in some areas of the healthcare system. The growing logjam leads to inefficient use of hospital resources, potential risks for patients being cared for by staff who are not trained in specialized long-term care needs, and increased emergency room wait times.

By making a number of changes, including assigning ALC patients to treatment streams, standardizing discharge practices, and working closely with the Mississauga Halton  Community Care Access Centre (CCAC) and the Mississauga Halton Local Health Integration Network (LHIN), Trillium reduced the number of ALC beds in its health centre to less than 7 per cent in March 2010 from eighteen per cent in March 2009 representing a one-third reduction in ALC patient days annually from 33,963 in 2008-09 to 22,715 in 2009-10. 

“By working closely with our CCAC and LHIN partners we were able to ensure a streamlined transition for patients from acute care to an appropriate community setting,” said Janet Davidson, O.C., president and CEO, Trillium Health Centre. “This not only reduces the average discharge time but also ensures that patients aren’t placed into long term care facilities inappropriately.”

Trillium will continue its efforts to refine patient discharge protocols, roles and procedures to improve the transition from acute care to community care. The hospital also continues to work closely with the CCAC to improve its discharge processes and opportunities for enhancing partnership with the LHIN.
