How do I obtain a birth certificate?
Trillium Health Centre’s Health Records department does not issue birth certificates. To obtain a birth certificate, contact The Office of the Registrar General at 1-800-461-2156.
After you give birth, the hospital
or midwife will give you a form called a Statement of Live
Birth. This is your child’s permanent identity record
and is needed to apply for a birth certificate from The Office
