How do I obtain a death certificate?

Trillium Health Centre’s Health Records department does not issue death certificates. To obtain a death certificate, contact The Office of the Registrar General at 1-800-461-2156.

A Medical Certificate of Death is completed by the hospital and is sent to the funeral home with the deceased patient. A Statement of Death is then completed at the funeral home with the family and the death is registered with The Office of the Registrar General.
