Leaving the Hospital
After a Stay
Discharge Information
A family member
or friend should take you home after a hospital stay. Please
make sure that you have after-care instructions, prescriptions,
medication, diet and follow-up appointment information. If
you are unsure about anything, ask the staff before you leave.
You may need care
after your discharge from the hospital. The hospital staff
will help you to decide on the available services that will
best meet your needs. The Community Care Coordination Department
will assist you in accessing services if they are needed.
Patient Accounts
You may pay your
account upon discharge, by cheque, cash or credit card (VISA,
Mastercard or American Express). For your convenience, some
patient units are equipped to handle your account payments
directly. Other units may direct you to our central Patient
Account office, located in the Main Lobby at Trillium - Mississauga
or to the Cashier's Office adjacent to the Urgent Centre Centre
at Trillium - West Toronto).
Hours for
both offices are:
Monday to Friday, 8:30 a.m. to 4:30 p.m. (holidays excepted)
Please remember to
bring your Ontario Health Card, Insurance Information, Hospital
Card (if you have one), and Credit Card.

Complaints
The staff, physicians and volunteers
at Trillium Health Centre are committed to providing you with
the best possible care.
If you have a compliment we would love to hear about it. If
you have a complaint about your care or the service provided,
please discuss this with a staff member or ask to speak to
the unit manager. Your feedback is important to us, so please
don't hesitate to let us know how we are doing.
If you have a concern, but have already left the hospital,
or if you are unable to resolve your concern with the unit
manager while you are at Trillium, please speak with our Patient

Health Records
If you require a
copy of your Health Record following a hospital stay, you
will need to contact our Health Records department. Before
we can release your personal information, we will need written
consent. Go to our Release
of Patient Information section of this site, to download
a copy of the consent form.
