Tools for Family
Physicians
The Department of
Family Practice at Trillium has identified these tools as
useful to family physicians. The Assessment Tools Group,
which consists of six family physicians from Trillium who
meet quarterly, has reviewed or developed all the tools.
Family physicians are invited to use these tools in their
clinical practice.

This toolkit provides a series of tools to be used by family
physicians to assess for memory problems in the primary care
setting.

Problem
Drinking Assessment

Diabetes Management
Centre (DMC)
Education in group
or individual format is provided for patients with Type 1
and Type 2 diabetes, insulin management, gestational diabetes
and Impaired Glucose Tolerance. A Healthy Foot Clinic also
available.
Hours of operation:
Monday - Friday: 8:00 a.m. - 4:00 p.m.
Wednesday: 8:00 a.m. - 6:00 p.m.
Referral Criteria
- Adults only.
- Only family physicians affiliated with Trillium Health
Centre may refer patients. a completed referral form with
Lab reports should be faxed to the DEC at (416) 259-5825.Referral
forms may be received by calling the DEC at (416) 521-4081
or by e-mail at DEC@thc.on.ca
Family physicians
will be mailed an Assessment Summary after the patient is
assessed by the nurse and dietitian at first visit. Family
physicians will receive a Follow-Up report after the patient
completes their series of classes and three month follow-up.
Family physicians will be notified if patients do not show
for their appointments.

Colorectal Cancer Screening Program
The Colorectal Cancer
Screening Program aims to save lives by having people 50 years
of age and older screened for colorectal cancer. Those at
increased risk due to a family history (one or more first
degree relatives with colorectal cancer) or with a positive
Fecal Occult Blood test test should receive a colonoscopy.
Please complete the attached referral
form for each patient and fax to the number below. You
may choose the physician you would like to refer to, or you
can choose the next available appointment. If you are referring
to a specific physician, specify the doctor and indicate if
you have contacted them directly.
