Best Practices
A user-friendly
resource tool based on best practice. It is designed to be
easily integrated into processes currently being used by the
Community Care Access Centre. The guidelines are intended
to facilitate discussion, collaboration, decision making and
mutual understanding between case managers and service providers
as they develop individual treatment and service plans.
