Follow-up Clinic
Trillium Health
Centre - Mississauga
Trillium's Cardiac Services Follow-up Clinic is the first service of its kind in Canada for cardiac surgery and cardiology patients.
Launched in December 2001, cardiac patients needing early follow-up care can be seen within three to ten days following discharge from hospital. Acute Care Nurse Practitioners, working in collaboration with cardiologists and cardiac surgeons, evaluate patient recovery and assess symptoms. They also provide education and interventions on pain management, wound management, and medications.
Cardiac patients have many concerns and experience a wide range of symptoms. Trillium established the Cardiac Service Follow-up Clinic to address patients' concerns in a timely manner and to provide additional time for patient and family education.
