Seniors' Health
Initiative
Trillium Health Centre's new Seniors'
Health Services include:
- Seniors' Health Clinic
- Seniors' Health Internal Consult Team
- Continence Clinic
A specialized geriatric
team provides assessment, consultation, education, treatment
and follow-up for frail persons over the age of 65 with complex
medical health care issues such as:
- functional decline
- mobility problems
- incontinence
- medication management concerns
- cognitive impairment.
The program is part
of Trillium's overall vision to develop a full range of specialized
geriatric services for seniors in our community. The population
is aging and the growth rate in the communities of Mississauga
and West Toronto is increasing above the provincial average. By
working together with our community partners to implement
patient-centred care services, the Seniors' Health Initiative
will address an important need in our community.
The goal of the
services is that with specialized, integrated and coordinated
clinical care, seniors in our community and beyond will experience
an enhanced quality of life and live more independent, healthy
lives.
Referrals to the
Seniors' Health Service are only accepted from Trillium-affiliated
physicians.
Seniors' Health
Clinic
Seniors who are
frail with complex challenges and are experiencing at least
one of the following are seen in the Seniors' Health Clinic:
- Cognitive impairment, confusion, falls, decreased mobility,
medication management issues
- Multiple and/or complex medical, functional and psychosocial
issues
- Two or more hospital admissions within the past year
- Frequent emergency room visits
- Urinary and/or fecal incontinence
- Chronic constipation
Seniors' Health
Internal Consult Team
This team provides
comprehensive assessment, consultation, recommendations and
follow-up to seniors admitted to an inpatient unit.
Continence Clinic
A Clinical Nurse Specialist assesses
ambulatory patients experiencing urinary incontinence, fecal
incontinence and/or constipation. A behavioural and educational
approach is used to assist people experiencing these issues.
The Continence Clinic
is located at Trillium Health Centre - West Toronto, 2nd Floor.
