
Complex Continuing Care Services
The mandate for Complex Continuing Care Services is to ‘care
for the persons whose medical conditions are such that a
multidisciplinary health care team is required to meet the
needs of the person.’
Individuals often have multi-dimensional problems requiring
medically complex care that may include ventilation, special
feeding systems, tracheotomy care, special pain management,
etc. The degree of support needed ranges from significant
to total, the result of a chronic or devastating illness marked
by impairments in either or both cognitive and physical function.
The Philosophy of
Care in Complex Continuing Care Services embraces a ‘Partnering
for Care Model’ with the recipients of care and with
their families. Complex Continuing Care Services offers 86
beds, with 54 at Trillium - Mississauga and 32 at Trillium
- West Toronto. The McCall Centre, managed by Extendicare,
is a partner in the provision of complex care, offering an
additional 120 beds to Trillium Health Centre clients at the
West Toronto location.
