Health Services Ambulatory Clinic - Toronto
The Ambulatory Clinic
provides comprehensive psychiatric assessments and case management
in the office for individuals who are 65 and over, who live
in south Toronto or Mississauga and who are at high risk of
requiring admission to hospital for psychiatric treatment.
Individuals under 65 may be considered, based on their symptoms
and/or issues. Follow-up home visits by a health professional
may be part of the assessment and ongoing treatment plan.
Referrals:
Referral by physician only. Referrals of individuals living
outside Mississauga or south Toronto can be made if the referring
physician is affiliated with Trillium Health Centre. This
service is not offered for people living in nursing homes.
