Geriatric Mental Health Services Mississauga Outreach Team

The Mississauga Outreach Team provides comprehensive psychogeriatic
assessments to elderly residents of Mississauga. The individual
must be age 65 or over with psychiatric symptoms related to
the aging process. This service is also available to younger
adults with symptoms relating to the aging process, i.e..
dementia. Service is provided in the person's place of residence
or in the outreach team office and includes consultation and
education for clients, families, caregivers and other professionals
involved with the individual. Specialized follow-up is available
on a limited basis. The client, their family and family physician
are informed of the findings and recommendations.
Referrals: Accepted from individuals, family members
and professionals. Must have a family physician involved to
implement the recommendations.
