North Peel ReLinC Services - Mississauga
North Peel ReLinC is a community-based case management service
for individuals in north Peel (Brampton, Caledon and Malton)
with a history of serious and persistent mental illness. Clients
served are aged 16 and older. The goal of the service is to
assist clients to obtain the skills and supports they need
to maintain and enhance their quality of life in the community.
Case Managers meet with clients in the environment of their
choice, including their home or elsewhere in the community.
Referral: Clients are referred by health care professionals,
family and community agencies. Self referrals are also accepted.
