Program
People sustain fractures
due to a variety of factors. One of those factors is osteoporosis
- thin, brittle bone structure that is unable to sustain even
minor trauma. Individuals who sustain fragility fractures,
typically of the wrist, shoulder, hip or spine, from low energy
forces (eg. a fall from standing height) have a high probability
of osteoporosis.
Trillium Health
Centre's Fracture Clinic identifies fragility fracture patients
as "High Risk Osteoporosis Patients". Family physicians
are sent a letter suggesting further investigation of their
patients' fragility fractures. Patients are provided with
a copy of the letter and requested to follow up with their
family doctor. These patients also receive an Osteoporosis
Education package.
The Osteoporosis Education Program
was initiated to help patients receive follow-up investigation
of their fractures with Bone Mineral Density tests, care and
further education in the community. Trillium's goal is
to prevent further low-impact fragility fractures in our patient
population.
