Trillium Health Centre's Fracture Clinic identifies fragility fracture patients as "High Risk Osteoporosis Patients". Family physicians are sent a letter suggesting further investigation of their patients' fragility fractures. Patients are provided with a copy of the letter and requested to follow up with their family doctor. These patients also receive an Osteoporosis
The Osteoporosis Education Program was initiated to help patients receive follow-up investigation of their fractures with Bone Mineral Density tests, care and further education in the community. Trillium's goal is to prevent further low-impact fragility fractures in our patient population.
