Who Can Refer?
Referrals are accepted
from:
- Trillium Health Centre specialists
- Non-Trillium specialists IF the client lives in the Trillium
catchment area – Mississauga or West Toronto
WSIB active claims
and WSIB Program of Care clients are also accepted for clients
who live in our catchment area.
How to Refer
Fax the completed
referral form to (416) 521-4062. We will contact the client
directly to set up their appointment.
Referrals are triaged daily, based on acuity and treatment
protocols, to ensure timely service.
Commonly treated diagnoses include:
Arthritis
- Amputation
- Burn injuries
- Carpal tunnel syndrome
- Complex hand and wrist traumas
- Contractures
- Crush injuries
- De Quervains Tenosynovitis
- Dupuytrens contractures
- Hand and wrist fractures
- Lacerations
- Nerve injuries
Scars
- Skin grafts
- Tendonitis
If you are referred,
please:
- Arrive 15 minutes before your appointment time to
complete registration
- Bring your health card
- Bring your reading glasses and a list of medications you
are taking (there will be forms to complete)
If English
is not your first language, please have someone call
to tell us ahead of time. It is helpful if you can bring a
family member or friend to the first appointment so we can
understand your problems and concerns. If that is not possible,
we will do our best to arrange for an interpreter to be present
at your appointment.
