Pharmacy
The Pharmacy Department provides medication services to both inpatients at the Trillium – Mississauga location and outpatients at the Oncology Clinic, Diabetes Education Centre, Pre-op Clinic, the Heart Function Clinic and the Cardiac Wellness and Rehabilitation Centre.
Prescription services for walk-in patients can be provided by an independent retail pharmacy in the Trillium – Mississauga location.
Pharmacists - Partners in Care
Pharmacists work alongside physicians, nurses, and other allied health care professionals, to provide the most appropriate drug therapy for each patient. Pharmacists monitor, review and evaluate medication therapy to ensure its effectiveness.Pharmacists work primarily in their assigned clinical area. They also provide patient specific care and support to other programs, including our aminoglycoside dosing program, Pharmacist Assisted Warfarin dosing Service (PAWS), renal dosing, and IV to PO programs. The pharmacists at Trillium also teach pharmacy students from the Faculty of Pharmacy programs at the University of Toronto and the University of Waterloo.
Pharmacy Technicians
Pharmacy technicians perform many of the medication distribution services. They are involved in Oncology, Clinical Trials, and Medication Reconciliation. The technicians play an important role in supporting the pharmacists in order for them to become an integrated member in the multi-disciplinary teams. Trillium is encouraging its technicians to pursue Regulation.
Patient Safety First
Pharmacists check to make certain that each patients’ medication is effective, does not cause unexpected effects, and does not react badly with other medications the patient is taking. The pharmacy team consults with the Institute of Safe Medication Practice to get the latest information on preventing medication errors.Pharmacists provide medication information to physicians, nurses, and other health care professionals on an informal basis or formally through presentations, in-services, or rounds. They also teach patients and family members about the safe and effective use of their medications while in hospital or after discharge.
