Seniors Health Services
Expanded Seniors’ Health Services Available NOW
We are pleased to announce new seniors’ health services at Trillium Health Centre for 2010. Since 2006 our services have included Seniors’ Health Internal Consult Team, Seniors’ Health Clinic, Seniors’ Health Outreach Team and Continence Clinic. The Mississauga Halton Local Health Integration (MH LHIN), as part of the Aging at Home Strategy, has provided funding to enable us to expand our services.
Expanded services include:
- an urgent geriatric clinic
- an expanded outreach team which serves the MH LHIN
- a falls prevention/bone health clinic and exercise/education program
Please refer your older adults to our Seniors’ Health Services.
Referrals are being accepted for persons 65 years of age or older experiencing at least one of the following:
- Geriatric syndromes e.g. cognitive/functional impairment, or medication management issues
- Multiple and/or complex medical, functional and psychosocial problems
- Falls or the potential for falls (specify Falls Clinic)
- Two or more hospital admissions or frequent emergency room visits within the past year
- Urinary incontinence, fecal incontinence and/or constipation (specify Continence Clinic). Please note that the continence clinic serves patients of all ages.
To arrange for an appointment at the Seniors’ Health Clinics or with the Outreach Team, a physician referral is required. The Seniors’ Health referral form is available on the intranet – Seniors’ Health or you can obtain a copy from the program secretary at 416-521-4090.
Please fax the completed referral form to 416-521-4116 so that an appointment can be arranged as quickly as possible.
