Building upon a well-earned reputation for clinical excellence and quality performance, we will do everything possible to achieve quality in all that we do.  

Achieving quality means that we consistently deliver exceptional clinical outcomes and exemplary patient and family experiences using evidence-based practices.

Trillium Health Centre's Quality Improvement Plan sets out clear goals and targets for improvement in 2011-12, along with an approach and specific changes that will be implemented to drive that improvement.

Patient Safety Indicators

The Credit Valley Hospital and Trillium Health Centre is committed to patient safety. Every member of our hospital – staff, physicians and volunteers – plays a role in protecting the lives of our patients through education and hand hygiene, process improvement and system resdesign.

Together, we are improving the safety of our patients.

Corporate Performance Indicators

At Trillium Health Centre, we are always looking at how we are performing and use different performance indicators to get a sense of where we excel and where improvements could be made. This information is essential to Trillium’s commitment to providing quality and safe care for its patients.

Our Board of Directors and Management Team believe it is important that the community we care for has access to this performance information.

The information on this site is intended to be a short, practical resource for anyone interested in the indicators Trillium uses to monitor and improve its performance, practices or outcomes as they relate to:

  • Hospital Standardized Mortality Ratios (HSMR)

The methods we use for making comparisons changes for each set of data. Where possible, we report our performance in relation to provincial and national hospital targets or averages.
