Quality Improvement Plan – 2012/2013
In March 2012, the Board of Directors for The Credit Valley Hospital and Trillium Health Centre approved the annual Quality Improvement Plan (QIP) for 2012/2013. While this is the second year in which hospitals have been required to submit an annual QIP, it is our first QIP as a newly merged organization. It is one single plan for the hospital, presenting a common set of corporate targets across all sites.
The Quality Improvement Plan reflects our ongoing commitment to deliver the highest level of safe, quality patient care while also being in accordance with the Excellent Care for All Act, 2010 (ECFAA). This legislation requires all hospitals in the province to submit an annual QIP with the purpose of helping hospitals to further improve the quality of safety for the care we deliver.
The QIP presents our high priority goals, associated indicators and targets for the year ahead as well as the activities we will undertake to drive changes needed to achieve each of these goals. In shaping this plan, we have taken into account the growing health care demands of our community due to population growth and the increasing number of patients over the age of 75 in need of acute care, as well as the fiscal challenges facing the public sector as a whole.
Our Quality Improvement Plan reaffirms the benefits of the merger, articulates our philosophy of quality, and emphasizes the importance of achieving a consistent quality health care experience for patients in the Mississauga and West Toronto communities.

Quality Improvement Plans – 2011/2012
The Quality Improvement Plans for the previous year (2011/2012) and the corresponding Progress Reports below were individual plans for Trillium Health Centre and for The Credit Valley Hospital. These plans were developed before the merger; however, they illustrate a common commitment to quality. It is this common commitment that provided the foundation for the integrated plan developed for 2012/2013.
Our annual Quality Improvement Plans help us to identify areas requiring improvement and to evaluate how we are doing in reaching our goal from year to year. We continue to be proactive in our focus on providing the safest, highest quality patient care possible.

Patient Safety Indicators
The Credit Valley Hospital and Trillium Health Centre is committed to patient safety. Every member of our hospital – staff, physicians and volunteers – plays a role in protecting the lives of our patients through education and hand hygiene, process improvement and system resdesign.
