About Us
The West GTA Stroke
Network is one of eleven regions across the province which
comprises the Ontario Stroke System (OSS). The region covers
an 1800 square kilometer area and services over 1.5 million
people in the Central West and Mississauga Halton LHIN's (Region
of Peel and parts of Etobicoke and Halton).
Included in the
network are all hospitals, Community Care Access Centres (CCAC),
Long-Term Care Homes (LTC) and community organizations/agencies
that provide services for stroke survivors and their families.
Trillium Health Centre is designated as the Regional Stroke
Centre and provides secondary stroke prevention services through
its Regional Stroke Prevention Clinic.
The
West GTA team collaborates and partners with stakeholders
across the health care continuum to identify, develop, implement
and evaluate initiatives based on evidence and best practices.
The goal is to decrease the incidence of stroke and improve
patient care and outcomes for stroke survivors and their families.
